Hubbard Care Center is contracted with Millennium Therapy to provide Physical, Occupational, and Speech Therapy to our residents and to the community through appointment. Our rehabiliation services are designed to help each person achieve and maintain the highest level of self care and independence. Restorative care continues after the formal therapy as an ongoing service through exercises and games.
Our facility provides 24-hour professional care. Each resident is treated as an individual, focusing on their physical, emotional, social, and spiritual needs through a comprehensive care plan with the resident, family, and all staff of the Interdisciplinary Team.
